She recently joined a gym program called Rhumba dancing lessons. What is the appropriate recommendation for KC’s use of contraceptives?
  1. A. Recommend KC to start nicotine replacement therapy and concomitantly, can start combined oral contraceptives
  2. B. Since KC is a heavy smoker, hormonal contraceptives are not recommended and therefore, suggest non-hormonal contraceptive methods.
  3. C. Use low dose 20 mcg estrogen-containing hormonal contraceptives.
  4. D. Recommend levonorgestrel/ethinyl estradiol vaginal ring.

Tips: In women who are 35 years of age and older, heavy smokers (smoking >15 cigarettes daily), hormonal contraceptives are completely contraindicated. Based on KC’s current status, she is not a candidate for hormonal contraceptives. On the other hand, women over 35 years and smokes less than 15 cigarettes can be considered for low dose estrogen-containing or progesterone only contraceptives.
